As of a couple updates ago, it appears that the unplayed count doesn’t update in real time anymore.

I recorded a 30 second clip using az screen recorder and this app doesn’t allow video to be uploaded. I did notice while trying to figure this issue out that playing with the “show played” switch on the top center of the settings page possibly corrects the issue.
